The PCR (stands for Polymerase Chain Reaction) is a basic laboratory test for diagnosing active SARS-CoV-2 infection. It is currently the reference technique because of its sensitivity and specificity.
For best results, nasopharyngeal swab samples are used, and the technique allows the virus to be detected before symptoms appear.
Several fragments of the genetic material of the coronavirus (its RNA) are localized and amplified using PCR. If virus RNA is detected after the laboratory analysis, the result will be reported as positive and confirm the presence of an active infection.
It is a complex test that requires qualified personnel and special equipment to perform it.
This test detects specific proteins that are present in the virus itself. It is therefore also a test for the detection of an active infection, but with a lower sensitivity, as there is no prior amplification of the biological material, as is the case with PCR.
As with PCR, the sample is a nasopharyngeal swab.
It is a simple test that can be performed outside laboratory facilities, but always by professionals, and the results are rapid, available in 15 minutes.
This test is recommended for symptomatic patients who have been symptomatic for less than 5 days.
ELISA and CLIA tests are automated high-throughput assays for the quantitative or semi-quantitative determination of IgG and IgM antibodies against SARS-CoV-2 in blood samples. This means that they are not used to detect an active infection, but to detect our body's immune response.
IgM antibodies can be detected shortly after the incubation period (on average around day 7) and remain in the blood for a short period of time. If the levels of IgM in blood samples are positive, this can be an indicator of an acute infection and would have to be confirmed by PCR technique.
IgG antibodies appear later (on average from day 10-14) and circulate in the blood for a longer period of time. They are usually an indication of a past infection that has been overcome.
To reduce the rate of infection, it is important that a large proportion of the population is immune, either because people have acquired antibodies naturally after infection or because they have been vaccinated. Therefore, knowledge of immunity (antibody levels) using high-throughput techniques, such as chemiluminescence, is an important prerequisite to reduce the incidence of the virus in new waves. Thanks to the specific antibodies, our body recognises the invading virus and fights it to prevent the disease from developing in the future. As it is not yet known how long the specific antibodies to coronavirus persist, regular checks may be needed to see if immunity is being maintained.

*Results between 0.9 and 1.1 are considered indeterminate. You are probably in a phase of seroconversion (change in antibody level). Please consult a doctor for interpretation of your results. It is recommended to repeat the antibody test in one week.

The antibody rapid test, like the ELISA or CLIA test, also provides information about the presence of IgG and IgM antibodies against SARS-CoV-2. However, in this case the result is qualitative. As in the previous case, it is also not used to detect an active infection, but to detect our body's immune response.
At Juaneda Hospitals, we offer these tests to detect antibodies to people who want to know if they have been infected with the coronavirus, whether or not they have had specific symptoms. Knowing whether the infection has already been overcome is very valuable information in seroprevalence studies. These studies tell us what percentage of the population has passed the disease.
The advantages of these rapid tests are (as the name suggests) the speed of the results and the possibility of using them outside laboratory facilities.

According to the Department of Health, "there is published evidence of the formation of neutralising antibodies, both in animal models and in human cases". However, at present we cannot say that a person who has already overcome the disease and formed antibodies is safe from re-infection. Based on the studies published so far, we can indicate that there is some degree of protection, but it does not appear to be complete immunity in all cases.

In order to reduce the number of infections, the immunity of the population is important, either because people have formed antibodies after infection or because they have received the vaccine. In this sense, the pandemic has left a large number of people who have contracted COVID 19 with sequelae that affect their daily lives. After overcoming the disease, patients suffer varying degrees of respiratory disorders (pulmonary fibrosis, dyspnoea), muscle and joint disorders (pain, weakness, atrophy), nervous system disorders, cardiovascular system disorders and cognitive disorders (language, attention, learning).
This is why we have set up the Post-Covid Unit at Juaneda Hospitals. It is intended for all those who have overcome the disease, regardless of the severity, from asymptomatic (knowing only from a positive PCR) to those who have been admitted to the intensive care unit. The unit is designed to: carry out checks, detect and treat secondary diseases or determine antibody levels to the virus.
The antibody levels, determined using high-performance techniques such as chemiluminescence, will enable us to reduce the incidence of the virus. Antibodies are substances that appear in the blood 4-6 weeks after contact with the virus. We find people who do not generate antibodies at all, those who do not generate enough antibodies and those who generate a significant amount of them. In the last case, we speak of immunity, as the antibodies formed fight against the invading virus when contact occurs again, thus preventing the disease from developing in the future.
After overcoming the disease, it is important to monitor the patient's health and to analyse and classify the possible consequences that the virus has caused in the body and that may eventually lead to chronic problems. Depending on the severity of the disease, specialists from different fields are involved.
